Field Trip Assumption of Risk, Release of Liability & Participation Agreement

This waiver applies only to school, organization, or group field trips at Becky’s Mindful Kitchen (“BMK”).

By allowing a student to participate, I acknowledge and agree to the following on behalf of myself and/or the minor listed below.

Activities & Environment

I understand that a field trip at Becky’s Mindful Kitchen may include, but is not limited to:

  • Interaction with animals, including goats and chickens

  • Outdoor activities in wooded and natural environments

  • Uneven terrain, trails, weather exposure, insects, and wildlife

  • Use of outdoor features such as a treehouse, fire pit, archery range, swings, slack lines, games, and activity areas

  • Optional food-related activities, including no-bake food activities, food assembly, or demonstrations

  • Indoor access to Becky’s Mindful Kitchen for bathrooms, water, or scheduled activities

I understand that activities may vary based on age, group size, weather, and safety considerations.

Assumption of Risk

I acknowledge that participation in a field trip at Becky’s Mindful Kitchen involves inherent risks, including but not limited to slips, falls, scratches, minor injuries, allergic reactions, burns, illness, or other unforeseen incidents.

I understand that animals are unpredictable and that outdoor and farm environments carry natural risks.

I knowingly and voluntarily assume all risks, both known and unknown, associated with participation, even if arising from the negligence of Becky’s Mindful Kitchen, its owner, staff, volunteers, or agents, to the fullest extent permitted by law.

Food & Cooking Activities

I understand that some field trips may include a short, hands-on food activity or food demonstration.

I acknowledge that:

  • These activities are educational and age-appropriate

  • They may involve shared spaces, food handling, or common ingredients

  • Becky’s Mindful Kitchen does not guarantee allergen-free environments

I accept responsibility for informing teachers or chaperones of any food allergies or dietary restrictions. Participation in food activities is optional.

Supervision Responsibility

I understand and agree that during field trips:

  • Becky’s Mindful Kitchen provides education, instruction, and guidance

  • BMK does not provide child supervision

  • Supervision remains the responsibility of teachers, school staff, and designated adult chaperones at all times

Rules, Conduct & Participation

Participation in the field trip is contingent on following posted rules and verbal instructions provided by Becky’s Mindful Kitchen staff.

I acknowledge that students who do not follow rules or instructions, engage in unsafe behavior, or disrupt the experience may be restricted from participating in certain activities or from the remainder of the field trip.

No refunds or credits will be issued due to behavior-related limitations or removal from activities.

Release of Liability

In consideration for participation, I hereby release, waive, discharge, and hold harmless Becky’s Mindful Kitchen, its owner, employees, contractors, volunteers, and agents from any and all claims, demands, actions, or causes of action arising out of or related to participation in the field trip, including personal injury, illness, property damage, or loss, to the fullest extent permitted by law.

Medical Authorization

I certify that the participant is physically able to participate in field trip activities.

In the event of an emergency, I authorize Becky’s Mindful Kitchen to seek emergency medical treatment if necessary and accept responsibility for any associated medical costs.

Photography & Media

Photography or video may be taken during field trips for educational or promotional purposes. Participation in the field trip does not require photo permission, and consent is indicated separately on this form.

Acknowledgment

By signing below, I confirm that I am the parent or legal guardian of the minor participant, or otherwise authorized to sign on their behalf. I have read and understand this agreement and sign it voluntarily.
